No change in setup is necessary. Just a flexible embouchure. Most
clarinetists do not try to gliss over the break, but start the gliss around
third space "c". One technique is to pull ALL the fingers off the open tone
holes at the same timeas you hit the "c'", while controlling the ascent in
pitch with throat and embouchure.

A small hesitation right before centering on the high "c" is very
effective!!!!!!!!!

If you open up and relax, it's not that hard. Too tight and you'll never do
it.
