I actually answered my own question with a little bit of thinking. I
looked at her reed and she was playing on a Rico 2 (argh!!).

I put her on a Vandoren 3 and she played perfectly in tune. We are
going to work an arching the tongue so she can actually play a little
sharp. With all pushed in she is almost perfectly in tune on almost all
notes now.
