The good repairpeople have reamers for clarinet bores so they can be put back to spec; I've seen Francois Kloc's, so I know that they exist. Since you say there's a buildup I'm assuming the bore is smaller than it should be.

The barrel could also be reamed or replaced; barrels are not that expensive an item.
