Wendy--
I used to have that problem, and occasionally still do.  It's caused by the
mouthpiece and reed.
Try making sure you can see your reed's tip just a little above the top of the
mouth piece.  Make sure your ligature is not "choking" the reed.  There should
be a little line on the moth piece, make sure your ligature is always under it. 
Also, when you go into the upper octaves don't change your embouchure.  It
should never change.
