1. Get with a good teacher and be sure that your hand and wrist positions
are correct. Bad placement leads to pain and even long term problems.

2. Use a neckstrap to help support the weight. These ARE available for
clarinet. There are professional musicians who use them so don't worry
about what other people may think of you. These are a BIG help. The thumb
and wrist is not really meant to carry this type of load and it is a wonder
that more people don't have problems.

3. There is also a stand available that can be used to support the clarinet
while playing.

4. See a doctor who specializes in treating musicians as you may be
developing permanent problems. If you can't find a doctor who treats
musicians, find one who treats athletes. A doctor who does not specialize
in these areas may tell you to quit playing and I doubt that is what you
really want.
