I made one once for one of my clarinets. Didn't work out because I kept
forgetting it was there. If I recall, one of the side benefits was that it
gave me a good G#/A# trill by fingering G#, moving first finger left hand onto
the bis key and trilling the middle finger.

The little arm is attached to the pad cup arm of the D#/A# (Eb/Bb), which is
part of the bottom ring key on the upper joint. First finger of the left had
skoots over to both close the hole and lower the top ring key, and at the same
time keep the pad below the first finger closed. Saxophones have this key, and
oboes have similar configurations, though I think they accomplish slightly
different tasks on oboes.
