As far as I know they mean one and the same (or at least that's how I've
always interpreted them...) I've played a lot of Mahler and had to deal
with this several times. When I do this I raise the instrument to being
almost level, usually slightly off to the side of the stand (so my bell
isn't right in the stand). I do it by changing my embouchure slightly and
bending my head back slightly. I don't play with a neck strap so I don't
have any weight on my neck. Therefore, I've never had any pain.
