I got mine in 1977! Looking further, the bottom of the cover says "Revised
edition, The Institute for Woodwind Research" c1977." The forward is as
follows:

This study grew out of a desire to know what was occurring in clarinet
pedagogy throught the United States and Canada. In 1966, when the author
wasas a student at the University of Nebraska, Lincoln, the graduate
clarinet class undertook a survey of equipment and studies used by
professional clarinetists and teachers in the U.S.A. While this study was
enlightening, the author felt the necessity for an additional survey to be
run in 1971-72 that concerned a larger scope of clarinet pedagogy using the
questionnaire procedure.

This second revised and enlarged survey was mailed in the summer of 1971,
followed by a second mailing in January 1972. Of the 250 teachers and
performers who were sent survey forms, a total of 162 replied.

The whole work is very interesting, but its style makes it look like part
of a master's thesis or doctoral dissertation.
