The hole can look perfectly clean and so can the pad. This hole is small
enough that just a light coating on the walls of the hole can do it. The
only way you can tell if it is dirty is to take off the register key and
clean the hole with something like a pipe cleaner through the hole and see
if it comes out clean.

I keep my clarinet perfectly clean too and always store it in its case when
not playing and still have to clean this hole every now and then as my
clarinet is particularly sensitive.
