Something that is quite possibly being over looked here is muscular tension,
not just a repetitive stress injury. I had chronic problems with pain in my
wrist and neck to such a degree that caused me to quit playing for nearly
10 years. This pattern disappeared after my instructor and I analyzed my
breathing and posture while playing. Often, raising one's shoulders even
slightly when breathing rather than expanding the abdomen is the cause of
the problem. The strain on neck, arm and wrist from this action is subtle,
but over time can, and for me , did lead to the sort of discomfort that you
describe. I do understand that the wrist and thumb can be inflamed by the
strain of simply supporting the clarinet, however, I thought this
possibility was worth mentioning. I have not used a neckstrap since my
instructor and I remedied my poor posture and breathing habits. Regards
