You would not expect "do" in an ascription of authorship - the genitive
case is more likely. My Polish is elementary, but there is a long poem by
Mickiewicz called "Konrad Wallenrod". It seems likely that the
piece referred to is an arrangement of an extract from a longer work
inspired by this poem, or, possibly, from incidental music to a dramatic
version of it, if there is such a thing..
So a reasonable translation would be:
Extract from the music to "Konrad Wallenrod".
