I have always heard ( and played) f#-g#-a#. . .the a# carries over from
earlier in the bar. This makes more sense to me than the chromatic
substitution you mention. Keeps in the whole-tone pattern of the thing. On the
other hand, the only person besides me I have heard play the piece is John
Mohler (monster performance). He had an arrangement for band that he used to
do with Revelli.

Can't help you with the composer questions. Would be interested to know
myself.
