I have taken the thumbrests on my R13 and 10G and inverted them so
that my thumb is higher in relation to the rest of my hand than it
was before. You just take out the two screws holding the thumbrest
on and put it back on upside-down.

The curve of the thumbrest is different then, but if you use a
cushion this shouldn't matter much (i use a piece of rubber tubing).

Also, there are some older clarinets on which this won't work because
of the way the thumbrest is made (i.e. one screw above and one below,
rather than side-by-side). But i think for many modern instruments
this should work, giving you maybe a half-inch of difference.
