The Rehfeldt book is available at Amazon.Com. I've been amazed at the breadth
of their coverage.

Once I got it, I realized it wasn't really what I wanted; I was looking for a
good discussion of altissimo techniques and alternative fingerings, but most of
the Rehfeldt book is invested in multiphonics and subintervals (quarter-tones
and less).
