After having made two complete case interiors, I can give you my experiences on
the subject.

I use standard 2" building styrofoam insulation. (The blue kind). It comes in
huge sheets, but is quite cheap. It is much better than the styrofoam with
little "balls" mentionned before. It can be shaped, cut, drilled, etc. I cut
the piece to the size I want for the case, and then, using a rotory tool with a
grinder type attachment, I shape it to the size I need to fit the clarinet. It
is also quite messy, but not nearly so as the other type of styrofoam.
It's quite dense, and so provides great "crash" protection, and living in a
cold climate (Montreal) I found that my clarinet was insulated FAR better than
in the original case. It would still be warm after being out in sub-zero
temperatures for a while.
As someone mentionned, the hard part is covering with material. Velvet is not
that expensive. For what you'd need to do an entire case it's only a few bucks
(around 10). I've tried different methods and am not satisfied with either,
but you can look at other cases and see what they do. There's the just using a
single sheet of matieral and trying to make all the folds look decent, (which
is quite tricky and frustrating) and I also tried doing each hold separately
(which was quite easy) and then making a cover piece of cardboard (or similar)
and cutting holes in it to fit around the holes in the bottom. That worked
alright, but it too had its drawbacks.

If anyone needs anymore info on this process of making cases, or wants to buy a
ready-made insert for a single clarinet (just add box) fitted for a selmer
prologue, but probably would fit any Bb clarinet just fine...
