And then there are the instances when the wrong clarinet is played, and
the performer is oblivious --

I went to a performance of the Stravinsky L'Histoire du Soldat Suite
(trio version for violin, clarinet and piano). For the uninitiated,
this is written for A clarinet -- except for the last movement, which is
written for B-flat clarinet.

In this performance, however, the clarinetist did not change clarinets
before the last movement. I thought -- oh, she's going to transpose --
but she didn't. I talked to her after the concert about this, and it
turned out that she had missed the cue to change to B-flat (which is not
that small -- at least in the edition she was using). At this point,
the pianist commented that that must have been the reason that the piece
did not sound like the recordings that they had been listening to while
preparing the piece.
