When I studied with Joe Allard in the late '60's/early '70's, he had
played with false teeth for a long time. He had two thumb rests
connected by a piece of rubber tubing, so that he could use the right
thumb to "pull" the clarinet against his lower lip. Of course, he also
played sitting down.
