Jason,
I don't know why it didn't happen on your plastic instrument (I
can think of a couple of reasons which are not germane to this post) but
the reason that screw work themselves loose is that either the screw
threads or the post threads are stripping little by little. Time to go to
a repairman who will either re-thread the post and/or put in a new screw.
