In my collection are clarinets with three different mechanisms for
dealing with throat Bb. 1. THe double register key works really well, I
don't know why it isn't standard. 2. THe McIntyre system uses a
properly sized and located hole for Bb and reserves the thumb key for
12ths. And 3., the wrap around thumb key eliminates the tube in the
bore and produces a wonderful Bb with no added mechanism.

Why clarinettists tolerate throat Bb is beyond me, we oboists have a
cure for our equivalent, low F.
