I really recommend a neck strap. If used right (and you do have to get
used to it), it takes a lot of weight off the thumb. I use one whenever
I expect to be playing steadily for more than 20 minutes or 1/2 hour.
My thumbs are no longer strong as I popped the joints too many times
from incorrectly executed martial arts techniques. Children really
should use them too as they are not strong enough and they often start
"biting" the mouthpiece to hold the clarinet a little steadier. The
only reason that it is not a bigger problem for children is that they
normally don't put in as much playing time at one sitting.

Several adults of my acquantance used them regularly. I was watching
the Metropolitan Opera broadcast of La Forza Del Destino last spring and
the principal clarinettist was using a neck strap (and he was pretty
good sized fellow).

I have tried the Clarichord from the Woodwind catalog and it works well
(it is elastic and adjusts via velcro). The elastic does wear out so
you have to replace it every now and then. It slips over the thumb
rest. I've heard of another that fits around the middle joint, is
adjustable, and is not elastic but I have lost the company name and
address.

In my opinion, the clarinet is perhaps on the borderline as far as
supporting the weight without a strap.
