I think that this is a problem that needs to be addressed and discussed
with your clarinet instructor. It might not mean that you need physical
therapy it might just be a simple problem with your hand placement that
needs to fixed.I remember having a friend having an aching problem.
On the other hand(please correct me if I am wrong) it seems like your
parents are forcing you to practice three hours every session. From my
opinion I would go completely insane if my parents did that too me. One
time I did 6 hours of practice in one day, and all I got was aching hands
and a major migrane, while another time I practiced two hours in one day
and I figured out a problem with my embouchure. I guess what I am trying
to say is that practicing a technique the right way for two hours per day
is more important then practicing a technique the wrong way for 6 hours
per day, regardless if it is all at once or in 2 o 3 sessions( I am not
criticizing your practicing method). If you are having a major frustration
over something, I would stop for awhile and if it continues, stop
practicing for the day. This can lead to many achings.
I hope I understood you and I hope this helps ::)

Happy Practicing!
