Sorry to break this to you, pal, but the only way I've solved your
questions 1,2, and 3 is by lots of practice. The secret to your common
problem, my Klarinet friend, is just how to.

This is what I'd do:
One of the best ways to improve a note above the break is a good
embouchure. The OCR gives a good article on this, the address is

The next thing you should be conscious of is where exactly is your tongue
when you play. It should be in a floating, neutral position midway in
the back of the mouth.

Finally, use only the tip of the tongue for staccato. Think of a "tee"
when you tongue. Practice with all this in mind to a metronome at a slow
tempo with straight quarter notes.
