For around 20 years, I have been using a lip guard made by a
dentist/tenor sax player friend of mine. I took him a copy of an article
written for the Selmer Bandsman (I think) by Rosario Mazzeo (remember the
Mazzeo clarinet?) who was a long time member of the Boston Symphony. He,
also used one of these lip gadgets, and the article told how to make one.
I no longer have the article, but basically, the dentist took a mold of
my upper and lower teeth, and made the device out of the same material
that they make false teeth from. I had him make it perfectly smooth (he
offered to cut fake teeth lines in it, but I wanted it smooth. The price
2o years ago was $75. I never play without it!
