I personally cannot play without a neckstrap anymore. I used to have a
lot of pain in my thumb, wrists, and forearms before I started using a
neckstrap, but I don't have those problems anymore. The only thing that I
have found is that I didn't like the BG neckstrap. There is too much
elastic. The purpose of a neckstrap is to take the weight off of your
thumb, not so the clarinet can jump around all over the place. I use (I
forget what brand it is) the neckstap that is about an inch wide of black
elastic. I am finding that the elastic is progressively getting stretched
farther and farther, and soon I going to run out of velcro to fasten it
with. Otherwise, it doesn't take enough pressure off of my hands.

Something another clarinet player did at my school that had the same
problem as me, is to take a sax neckstrap and attach the bottom of the one
that I mentioned in the above paragraph (the part that attaches to the
thumbrest)to the sax neckstap, replacing the sax clip. She did this
because sax neckstraps don't have the elastic, and that way they stay in
one spot, and don't stretch out. I might have to do that soon.
