Yet another option that I have used is to by a hockey mouthguard, slice
a little piece off, dunk it in boiling water for 30 seconds or so, and
then take it out and mold it to your teeth.

I paid a whole $0.97 sents for one, and have made several (because I
keep losing or misplacing them!), and stil have half of the mouthguard
left!
