I believe you are referring to the "Three Etudes on Themes of Gershwin"
by Paul Harvey, a noted woodwind pedagogue and composer from Great Britain. It
has finally been republished and is available from Emerson Editions (in,
I believe, Ampleforth, England) and is catalogue #177
