I had this problem when I was a younger player. My teacher told me to fix up
my embouchure, that is tighten it up! And sometimes the reed is to blame.
Your reed is maybe too hard for you. My teacher told me that the mouthpiece
must not wiggle around. The best embouchure is when it's firm and tight. So
the mp is actually resting in there and can be taken out but can not be
pushed around. I'm not really sure how to describe it, I would have to show
you, if you don't know what i mean. Change your reed strength to about half
or one softer, Not to show off or anything my throat Bb is just as beautiful
as any other note on my beloved clarinet.
