You could try Luyben Music, or Eble Music, who might stock old copies of
the book. Also try Albert Alphin Wind Instruments (617) 449-4197, or
Muncy Woodwinds (800) 333-6415.

"A Book for the Clarinet Reed Maker" by Ronald Vazquez is availble from
Muncy and Alphin, and Alphin has my personal favorite, "Reed-Mate Reed
Guide" by George T. Kirck. Glenn Bowen also had a book, "Making and
Adjusting Clarinet Reeds", published by Sounds of Woodwinds. I don't know
if it's still available.
