To go along with Don Yungkurth's suggestion about reading ahead while sight
reading-

I had a clarinet teacher who cut out a window in a 3x5 card and would place
it on the music as I was playing. It forced ne to look ahead when I was
playing since I didn't have very long to look at the stuff in the window. In
the beginning it seemed like I was continuelly behind- but my reading
improved as time went on.

The only drawback of this method is that you need to have another person
around to move the card on the music (obviously).
