I have found both. The Coast Guard Band library has an arrangement for band
which has already served me in a few performances. I couldn't tell you if it
is still available, but it was published by Carl Fisher in New York.

I did call them concerning the orchestra version, and it is available for
rental. They price the rental according to the size (budget) of the
performing group. For New Britain (CT) symphony they quoted $160. They
might also have the band arrangement for rental as well.

It would seem to me that this work is in public domain, and if you already
had a copy, you could perform it without the rental (performance right).
Does anyone on this list know anything about this?
