A number of people have recommended Tom Reidenour's excellent book. Another
valuable reference is Paul Drushler's "The Altissimo Register: A Partial
Approach". This is published by Shall-u-mo, PO Box 2824, Rochester NY 14626
(although I don't know about current availability). It is not as easy to
read as Tom's book (the title refers to its acoustic approach, rather than
narrative), but it is well worth the effort.
