In Phillip Rehfeldt's book New Directions for Clarinet there are a few pages
dedicated to this. (Rehfeldt, Phillip. New Directions for Clarinet.
University of California Press, 1994 Pgs. 72-74.)

According to this source F. Gerard Errante wrote a piece entitled "Souvenirs
de Nice" in 1975. It called for the mouthpiece to be placed on the lower
joint of an A clarinet. Later in this piece it is called for that the
player plays both A and Bb simultaneously by controlling the reeds with both
sides of the embouchere.
(Maybe Dr. Errante would go into more depth for us, he's on the list..)

There is also a piece like this by Bill Smith entitled "64" written for
"demi-clarinet" ( Bb clarinet with moutpiece on lower joint.) There is also
a fingering chart illustrated. If you don't have this book you should...
