I have had some luck avoiding pain with the bass clarinet by useing a guitar
strap, hooked over the left shoulder instead of a neck strap. The strap is
wider and doesn't bite into your flesh, and when you let go of the horn, it
hangs horizontally or at an angle instead of jamming the reed into your face.
It doesn't take muck extra pressure of the hands to keep the instrument
vertical for playing- worth a shot.
