Another neckstrap option is using a regular alto saxophone neckstrap and tying
a piece of shoelace or cord in a loop around one's thumbrest. The hook of the
sax strap fits into the loop of the string.

I used this arrangement when I developed inflammation of the extratensor
ligament (?!) between my right thumb and hand in my senior year. The
combination of antiflammatory drugs and the neckstrap helped me get through the
year.

I think Michael Norsworthy's suggestion is more valuable however; the position
of one's thumb in relation to the clarinet is probably the best way of
preventing overuse syndrome. Dr. Verdehr switched my hand position slightly
and I have had no problems with pain whatsoever even though I practice more
than I did a year ago. Neckstraps, drugs and rest are very valuable, but
seem more curative rather than preventative. I am wondering if anybody
familiar with Alexander technique would have input into this discussion.
