I've had the same problem.
Mine started when I played bass clar with a neckstrap last year. I did have
to switch over to the peg right at the end of last year because it was too much
for my hands to hold.

This year, I was in a word processing class, as well as all my ensembles.
When you add the ensemble time, practicing time, and word processing time
together, my tendons took a beating.

I ended up wearing cock-up splints for about three to four weeks, continuously
EXCEPT when playing.

I also laid off the practice time, and laid off the typing, which meant I only
READ and didn't REPLY to any KLARINET stuff.

I'm fine now, and hardly ever wear them. I will sometimes, for long papers
that I have to type.

I also have started using a Claricord neckstrap. I find no problem with the
feel on the right thumb.
