I am a registered nurse and an amateur clarinetist. Your first order of
business is to control the symptoms you are now experiencing. Motrin
(also known as ibuprofen) is an excellent medication to control
inflammation and pain. DayPro is in the same family as ibuprofen and
does the same thing. Rest is essential; there is no way to ignore this.
Your current commitments are not that important. You may be contributing
to a possible disorder far worse than you now experience. Use ice or
moist heat (whichever makes you feel better) for 20 minutes 3 or 4 times
a day until you are over this acute phase. Studies are suggesting
splints make no difference in controlling pain or inflammation. I don't
see how a neck strap will help. Hand position is important so excess
stress will not be placed on your wrist tendons (where the carpel tunnel
is) as you bend the wrist to conform to the instument. It would appear
by the other responses that thumb rest repositioning should be
considered, but do this AFTER you control your current symptoms. You
have to take care of this aggressively and you have to do it now.
