Rules
1. Application Fee -$25 (I.C.A. Member) $35 (non I.C.A.)
2. A photocopy of some form of Identification proving age of contestant
3. A seperate written statement, signed by the contestant, attesting that the
recording is the playing of the contestant.
4. A summer address and telephone number should be provided if different from
those used during the academic year.
5. Pleas note that no application form is required
