There is a book for flute which we use in our studios for all of the winds. The title is something like:

Exercises for Facilitating the High Notes on Flute---DS WOOD---pub Cundy-Bettony.

It works great for clarinet and also for the altissimo in saxophone.

We also use HEISIG clarinet mouthpieces which are easier to go up with.

On sax the HEISIG mouthpieces are much easier to go up with, and the HEISIG sax like the mouthpiece is designed to play altissimo quite easily. We have many other excercises to solve the very problems you mention, and anyone that wants help can call (831) 425-5658.
