I have an older oboe that used to cause me a lot of pain in my right
hand when I play. There are two problems with the oboe:
1. the right hand keys are too far apart for my hand given that the same
hand must also bear (almost?) all the weight of the instrument
2. the instrument is too heavy given how far apart I must pose my right hand
fingers.

Because the instrument is pretty old, it doesn't have a "ski jump"
extension on the RH 1st finger key. Finally I built an extension for the
key, and now it works just fine. I also use a neck strap. Good luck,
