I don't know anything about the risks of moving
it up or not, but they have adjustable thumb
rests for Lorees.

I have tendonitis, which is different, but to
keep wrist pain and whatnot from affecting
playing, I use a FHRED (the oboe support stand
thingy that clips onto the thumbrest) that has
worked incredibly well.
